This information is collected during your first appointment. It is recorded electronically using a Electronic Personal Record Form (ePRF)

Personal information such as height, weight and medical condition will only be shared with The 1:1 Diet by Cambridge Weight Plan and any Medical Professional as required.

Personal data is held and kept for two years after you leave. Any hard copies are securely disposed of after this time.
